On page 3 of the 1065 Tax Form, question 5 asks if the club
filed From 8893. Should this form be filed every year?

No. The general recommendation is that clubs DO NOT file Form 8893. If a club has filed the Form (current year or prior year) this box must be checked. Once filed, the Partnership Level election is in effect until the club takes positive steps to revoke the election.
