Registration Revoked

On our monthly broker statement, one of our stocks was
listed as "registration revoked". It is worthless.
How do I enter that into our club accounting?

Which stock is it? (We should confirm that "registration revoked" truly means worthless and not something like "transferred to pink sheets".) If it is truly worthless, just enter a sale for $0.
