has name of member, club name, other info--"opted out of
centralized audit" with the rest of the taxes?

If you are mailing your return, you include everything between page 1 of Form 1065 and the page near the end that marks the beginning of the withdrawal reports. Note that this page may not be present if your club didn't have any withdrawals in 2022.
