Just wondering does anyone have a standard membership/partner registration or information form for when a member joins the Club?
Our Club is 20+ years old and haven't had any new partners for several years..
Now we have a couple of perspective new partners. Is there any standard information your Club requires from new partners?
Is the Club required to maintain hard-copies or can the Club just enter the partner information in a database?

Well, we use bivio so there's a roster in there that would have the basic - address, phone number, email address. We do also have an emergency contact person list. Which I actually had to use recently to get hold of someone to sign some papers for our new brokerage application. Of course, you need to have some method of having the new member sign the Partnership Agreement. We also have a sheet that lists the expectations and responsibilities for prospective members. We have an 'emergency contact' form. (Years ago we called it a 'Beneficiary Form', however we found out if a Club Partner dies, the person listed on the form may not be the legal beneficiary).

Make sure you get new partner Social Security numbers before you let them join.
The are required for club tax forms. Some clubs have had issues at tax time with new members who don't want to provide them. This holds up tax filing for all the members of the club.

It qualifies their interest in sharing and past experiences with investing. Our club had close to 20 years history before becoming a model club. Our membership requires unanimous vote of the partnership. I don't ever remember not approving an applicant.

Attached is a copy of the Partnership Member Application form for ACM Model Club of the Inland Empire Chapter. Prospective members are required to attend three meetings in four months for consideration as a partner.
This application has been in use for at least 20+ years with the only updates to incorporate technology as it develops and applies. Seeing this, I think maybe something needs to be added to address the responsibility and commitment to continue to attend meetings even when they are held online. We have one member,
although they continue to send their investment requirement, has been MIA since we had to go online.
This document is available as a Word document with form fields so that it can be filled out from a computer. If anyone would like a copy in a .doc format, please let me know.
We ask to have the application by their third meeting so that members can evaluate the prospect and have a basis to ask questions. Usually, during the three qualifying meetings, everyone has an opportunity to visit to get acquainted and any questions answered. Everyone is welcome to attend meetings and participate in discussions without the requirement of being a member. There are no voting rights, though Also, we have no limits to the number of visits a person may attend.
