You'd enter this using the merger button on the Accounting>Investments page.
For each share of BRLI you owned, you received 2.75 shares of OPK. If you were owed any fractional shares, they were rounded up to a whole share. You'll need to make sure you add that in.
Effective date is 8/21/2015.
