Having a new member fill in a W-9 form so you are sure you have their tax reporting information is a very good suggestion.

In the sample operating procedures that we provide, there is a section which might give you ideas of requirements for new members for your clubs:

You can adjust these to suit your club operations if you'd like. It is a good idea for your club to spell these out and agree to them ahead of time. This avoids confusion when you actually have a new member to consider.

Other than a W-9 form, new members should sign addendums to your partnership agreement and operating procedures that shows the date they joined and the fact that they have agreed to their terms.

You'd also want to give the members a description of how to make monthly contributions and probably get an initial deposit check from them.
